Geisinger Health System offers the "ProvenExperience™" mobile application as an optional application to assist Geisinger patients in providing feedback about a recent experience at Geisinger. By using the mobile application, you are agreeing to the following terms and conditions. IF YOU DO NOT AGREE TO THESE TERMS, DO NOT USE THE MOBILE APPLICATION.
- In using the mobile application, you have the option of entering some information that is specific to you. Any personally identifiable information that you submit may be stored within the application so it can be used to display specific information. Such information may also be stored in other Geisinger data management systems for improvement purposes. This information is considered your personal information and Geisinger only will identify the information by individual or use the information in an individual context to process your request made in the mobile application. The information may be used in a non-identifiable manner to understand if this feature is used and how in order to make updates or enhancements to the mobile application.
- Refunds consist of money that you as a patient have paid or are paying towards the cost of your care and can be made if you did not receive the best experience at your visit or hospital stay. Refunds are only available for services received at Geisinger entities and only for the specific service for which you did not receive the best experience. Refunds do not apply for disagreements with your insurance over what is covered, or your share of cost (deductibles or co-pays). In addition, refunds may not be given in cases of disagreement with medical decision-making (e.g., choice of drug or treatment plan) or Geisinger’s refusal to provide or prescribe a particular medication (e.g., pain medication, antibiotics). You agree that if you select a refund via the mobile application, you will be contacted within 2 business days to process your request. If you have not yet paid the amount, a credit for the requested amount will be deducted from your bill. If you have paid the amount of your request, a refund for the amount requested will be granted.
- You agree that the mobile application is not a substitute for medical advice and you will contact your provider with any questions.
- The mobile application is a "Related Service" to Geisinger.org and you accept the overall terms and conditions for Geisinger.org found at http://www.geisinger.org/pages/terms-and-privacy/pages/terms-and-conditions.html.
- You may discontinue use of the mobile application at any time with no impact to other services or applications you receive at Geisinger.
- You agree that the law applicable to any claim or dispute relating to the mobile application or you use thereof and all activity and any dispute that might arise between you and Geisinger, its doctors, nurses, staff or other affiliates hereunder, regardless of where you reside, shall be determined by where you primarily receive medical care at a Geisinger site.
